Patient Information

As a courtesy to our patients, all primary and secondary insurances are filed for you by our office staff. However, charges incurred are the patient’s responsibility and it is up to you to make sure your insurance company pays within ninety days of service and your account is paid in full.

All of our physicians participate in the Blue Cross, Blue Shield of Alabama PMD program as well as Medicare and Medicaid. We accept assignment on these claims. This means that your insurance company will send payment for your charges directly to our office, however you will still be responsible for any co-pay and deductible charges that apply to your coverage. Most commercial insurance coverages are subject to deductibles and co-insurance amounts, but we will file these for you provided correct information has been given to our staff to verify your coverage. Please be sure our office has the correct information concerning the name of your insurance company and correct contract numbers.

If your insurance company has special requirements such as preadmission certification, second surgical opinions or prior approval for in office procedures, please inform us before your surgery or testing is scheduled.

For your convenience you can download the following forms, fill them out and bring them with you on your first visit.

One of our physicians is always on call after hours, and on weekends and holidays to handle emergencies. If you feel you have an emergency, or an urgent problem that cannot wait until the office is open, you can reach a physician on call by calling the office phone number any time, day or night.

If the office is closed, our answering service will answer and contact the physician on call, who will call you back. We request that you contact the physician on call only for emergencies, or urgent problems that cannot wait until the office is open. If you are unable to reach the physician on call in an emergency situation, we advise you to go to the hospital emergency room for initial evaluation. The emergency room physician will then contact the physician on call for our practice.

For routine prescription refills, or questions regarding medications, please call the office number Monday through Thursday between 9:00 a.m. and 12:00 p.m. and ask to speak to the nurse.

Our physicians WILL NOT phone in prescriptions or refills on pain medications after hours or on weekends when the office is closed and the physician does not have access to your office medical chart. Please call for refills on these medications at the hours stated above. If you have severe pain and need pain medications after hours, we advise you to go to the hospital emergency room.
